You have the SX Gene, a rare genetic mutation that causes women to become completely infatuated with you. In this open-ended scenario, you can go anywhere and do anything. No matter where you go, you'll be fending off hoards of sex-crazed ladies...though perhaps that isn't such a bad thing. The intro starts off with some NPCs you can interact with, but you can just ignore them and go anywhere you want.
